Community Health Systems, Inc. (NYSE:CYH) reported a third-quarter 2026 adjusted loss of USD 0.48 per diluted share, wider than a consensus estimate of a USD 0.29 loss. On a GAAP basis, the net loss attributable to stockholders was USD 86 million, or USD 0.63 per share, compared with net income of USD 130 million, or USD 0.96 per share, a year earlier. Net operating revenues were USD 2.757 billion, down 10.7% from USD 3.087 billion and below a consensus estimate of USD 2.80 billion.
Divestitures drove the revenue decline, with total admissions down 12.3% and adjusted admissions down 11.9%. On a same-store basis, admissions rose 2.2%, adjusted admissions climbed 3.6% and net operating revenues grew 3.8%. Chief executive Kevin Hammons said volume trends and cash flow improved from the prior quarter, but payor mix and acuity developed differently than anticipated in a challenging macro environment.
Earnings Drivers and Portfolio Changes
Adjusted EBITDA, a non-GAAP measure, fell to USD 280 million from USD 376 million. The company cited divestitures, an unfavorable payor mix, higher medical specialist fees and higher supplemental reimbursement program expense, partly offset by higher volumes and reimbursement rates. Non-patient revenue also dropped against a prior-year quarter that included a USD 28 million legal settlement. Income from operations dropped to USD 146 million, or 5.3% of revenue, from USD 243 million, or 7.9%. A smaller income tax benefit also widened the swing to a loss.
Through nine months, revenue was USD 8.547 billion, down 8.9%, and the adjusted loss was USD 1.15 per share against adjusted earnings of USD 1.20 a year earlier. Adjusted EBITDA for the period was USD 918 million versus USD 1.131 billion. During 2026 the company has divested its 80% interest in one hospital plus nine other hospitals, the latest effective October 1. It now operates 59 hospitals across 32 markets in 12 states, along with more than 800 sites of care.
Cash Flow, Debt and 2026 Guidance
Operating cash flow was USD 155 million for the quarter against USD 70 million a year earlier. Cash and cash equivalents stood at USD 155 million, down from USD 260 million at the start of the year, while long-term debt declined to USD 9.568 billion from USD 10.380 billion. Over nine months the company repaid USD 2.151 billion of long-term indebtedness, helped by USD 894 million of net investing inflows.
The company updated its 2026 guidance, first issued on July 22. It projects net operating revenues of USD 11.3 billion to USD 11.5 billion, Adjusted EBITDA of USD 1.225 billion to USD 1.275 billion and a net loss of USD 1.70 to USD 1.60 per share. It also projects capital expenditures of USD 350 million to USD 400 million and interest expense of USD 820 million to USD 830 million.
Numbers to Revisit
Four readings frame the fourth quarter. Adjusted EBITDA is first: with USD 918 million booked through nine months, the guided range of USD 1.225 billion to USD 1.275 billion leaves USD 307 million to USD 357 million for the final quarter, against USD 280 million in the third. Payor mix and acuity are second, since management named both as the pressure points. Same-store adjusted admissions are third, up 3.6% this quarter. Liquidity is fourth, with USD 155 million of cash, USD 9.568 billion of long-term debt and operating cash flow guided to USD 100 million to USD 300 million for the year.
