Alignment Healthcare (NASDAQ:ALHC) is under scrutiny Friday after a brokerage downgraded the Medicare Advantage insurer to Sector Weight from Overweight, according to a market report dated Oct. 30. The report did not cite a price target from the brokerage. Its reference price was $8.06, down 1.5%, but that is not a confirmed pre-market quote, so the stock's direction before the open is not confirmed.
The call follows third-quarter results released after Thursday's close. Revenue rose 36.7% from a year earlier to $1.36 billion, ahead of the $1.31 billion consensus, according to market data. The company posted a loss of $0.04 a share, in line with estimates, compared with a profit of $0.02 a year earlier.
The more important change was in the outlook. Management lowered its 2026 adjusted EBITDA outlook to "no less than $100 million," per market data, while saying it expects at least 20% top-line growth in 2027. In other words, the company is still growing quickly, but profit expectations are coming down, which is exactly the trade-off that makes analysts more cautious.
A stock that has already lost most of its value
Alignment shares have been under heavy pressure this year. Market data show a 52-week range of $6.01 to $25.12, with the 200-day Moving Average at $16.89 and the 50-day at $9.99. The Market Value is about $1.67 billion.
Several analysts had already cut targets this month. One brokerage lowered its target to $10 from $16 on Oct. 6, keeping Equal Weight; another cut to $9 from $22 on Oct. 12, keeping Neutral; and a third downgraded to Market Perform on Oct. 9. One bank stood out by reiterating Buy with a $12 target on Oct. 22. The market-data consensus is Hold, with six Buy and seven Hold ratings.
A law firm has announced an investigation into possible securities claims related to alleged inflated EBITDA, market data noted. Such announcements are common after big stock drops and are not a finding of wrongdoing. CEO John Kao sold 385,270 shares on Sept. 11 under a pre-arranged trading plan.
The business, the index and what to watch
Alignment Healthcare, which operates under the Alignment Health brand, sells Medicare Advantage plans to seniors and pairs them with care coordination, a 24/7 concierge care team and its AVA technology platform. It serves markets in California, Florida, Nevada and North Carolina and has been expanding for 2027 into new counties in Arizona, Nevada, Texas, North Carolina and California.
Quality scores are a strength. On Oct. 8 the company said six of its seven eligible contracts earned four stars or higher in the 2027 CMS Star Ratings, with three at 4.5 stars. Higher star ratings bring Bonus payments from Medicare.
Alignment trades on the Nasdaq and is part of the Nasdaq Composite. Health care has been a relative haven this week as investors move out of big tech; Nasdaq-100 futures were down 2.62% around 8:03 a.m. ET, per market data, while the Dow was flat. Medicare Advantage insurers, however, face their own pressures from rising medical costs.
Key risks include higher-than-expected medical utilization, government reimbursement changes, and the cost of entering new markets. Watch whether other analysts follow the brokerage, how the stock trades relative to its recent $6 low, and early enrollment data for the 2027 plan year. Medicare's annual enrollment period, which runs into early December, will be the first real test of whether the new county expansions and strong star ratings translate into membership growth that supports the 2027 revenue target.
