Q3 Margins Narrow Despite In-Line Revenue
Universal Health Services (NYSE:UHS) shares are indicated down 5.12% at $168.25 in premarket after third-quarter results that were mixed. Revenue of $4.73 billion, up 5.3%, and adjusted EPS of $5.22 were roughly in line with consensus of $5.15 to $5.31, but adjusted EBITDA of $624.6 million missed estimates of about $634.9 million.
Operating margin fell to 9.6% from 11.6% a year earlier. Net interest expense rose 46% to $56.3 million, which the company tied to the Talkspace purchase. The results included a $47 million Medicaid gain and a $26 million reserve increase tied to the tentative Cumberland litigation settlement.
UHS, headquartered in King of Prussia, Pennsylvania, is one of the largest U.S. hospital and healthcare services providers. Through its subsidiaries it operates more than 400 facilities, including about 30 inpatient acute care hospitals and roughly 346 inpatient behavioral health facilities, plus outpatient, freestanding emergency and ambulatory sites in the U.S., Puerto Rico and the United Kingdom. It reports Acute Care Hospital Services and Behavioral Health Care Services segments, and generated about $17.4 billion in revenue in 2025 under President and CEO Marc D. Miller.
Peer Selloff Sets a Fragile Backdrop
The results follow a sharp selloff on Tuesday, when UHS fell 4.52% to close at $177.32 from $185.71 after HCA Healthcare (NYSE:HCA) signaled flat 2027 earnings on lower Medicaid supplemental payments and more uninsured patients. The broader healthcare sector ETF is down less than 1% in premarket, so the move in UHS is stock-specific.
UHS has a market cap of $9.17 billion and EPS of $22.54, and the 52-week range runs from $140.08 to $246.33.
Operating Trends Offer Some Support
Acute care adjusted admissions rose 4.9%. Same-facility behavioral adjusted admissions rose 2.5%, and patient days rose 2.1%. Nine-month operating cash flow rose 14.9% to $1.48 billion. UHS has beaten EPS estimates in six of the last seven quarters.
Medicaid supplemental payment uncertainty remains, along with the 2028 federal Medicaid cuts.
Earnings Call Focuses on 2027 Payer Mix and Labor Costs
The earnings call is due this morning, and options imply a post-earnings move of about 8.5%, against an average move of 6.93%. Investors will look for commentary on 2027 payer mix, labor and operating cost pressure, and the Talkspace integration. The July plan called for full-year revenue of $18.50 billion to $18.76 billion and adjusted EBITDA of $2.61 billion to $2.72 billion, so any update to that outlook will matter.
