Axsome (NASDAQ:AXSM) shares are trading down 8.52% at $132.13, against a previous close of $144.43, even though the company reported third-quarter results ahead of estimates. The stock is near its 52-week low of $123.00 and Volume is running about 10% above average.

Axsome, based in New York, is a commercial-stage biopharmaceutical company founded in 2012 that develops therapies for central nervous system disorders. Its marketed products are Auvelity for major depressive disorder and Alzheimer's disease agitation, Sunosi for excessive daytime sleepiness in narcolepsy and sleep apnea, and Symbravo for acute migraine.

Revenue and Prescriptions Climb

Total revenue rose 52% year over year and 19% sequentially to $259.7 million, passing a $1 billion annual run rate. Auvelity net sales were $207.6 million, up 53%, Sunosi net revenue was $48.2 million, up 47%, and Symbravo net sales were $3.9 million, up 88%.

The net loss narrowed to $12.9 million, or $0.25 per share, from $47.2 million a year earlier, and the adjusted loss compared with an estimate of $0.69. Selling, general and administrative expense was $203.1 million, Research and Development was $50.6 million, and gross Margin was in the low to mid-90s. Cash was about $314 million, which management expects to fund operations through Cash Flow positivity.

Axsome has trailing twelve-month sales of about $777 million and about 1,300 employees, and it is reinvesting in commercial expansion and pipeline development. The stock carries a forward price-to-earnings ratio of 106.01 and trailing Earnings per share of negative $3.83.

Auvelity total prescriptions were about 302,000, up 40% year over year and 14% sequentially. In patients aged 65 and older, new-to-brand prescriptions grew 211% year over year, and monthly total prescriptions have risen about 78% since the Alzheimer's agitation approval. About 8,200 new prescribers were activated and coverage reaches about 89% of lives. Management believes Auvelity can reach at least $8 billion in peak annual revenue.

Why Investors Are Cautious

Management expects gross-to-net discounts for Auvelity and Sunosi to rise slightly in the fourth quarter on seasonality, and expects Auvelity's 2027 gross-to-net to resemble 2026. It gave no formal fourth-quarter or 2027 revenue guidance and declined to endorse consensus, and it flagged Medicare prescription abandonment in Alzheimer's agitation.

The Phase 3 binge eating disorder readout for solriamfetol slipped again, to the first quarter of 2027. Piper Sandler trimmed its price target to $235 from $238 and called the selloff unjustified, and the stock is down roughly 45% from its June high.

Catalysts Ahead

The AXS-12 cataplexy PDUFA date is May 1, 2027, with no advisory committee planned. A Phase 3 schizophrenia trial for AXS-20 is expected to start in the first half of 2027, while ADHD pediatric and shift work disorder Phase 3 results and AXS-05 smoking cessation results are expected in the second half of 2027. Phase 3 enrollment for AXS-14 in fibromyalgia should complete in the third quarter of 2027, with a possible NDA within about six months of topline data.

The quarter confirmed strong commercial momentum, but the stock is reacting to what the report left open, including forward guidance and the delayed binge eating data. The pipeline calendar through 2027 now carries much of the weight.
