Universal Health Services Earnings: Did a $5.22 Beat Mask Weaker Profit Quality?

Universal Health Services (NYSE:UHS) reported third-quarter results after the close on Tuesday, October 27, with adjusted diluted EPS of $5.22. That cleared the $5.15 estimate as screened, but it was below the $5.69 adjusted figure of the prior year. GAAP EPS was also $5.22, versus $5.86 a year earlier, because there were no adjustments in 2026.

Net revenues grew 5.3% to $4,733.4 million, while net income attributable to the company fell to $308.7 million from $373.0 million. The release did not publish new guidance ranges and refers to the 2026 operating results forecast first disclosed on Monday, July 27. That forecast called for net revenues of $18.501 billion to $18.762 billion and adjusted EPS of $22.28 to $23.65.

Same-Facility Trends: Where Did Volume Growth Run Into Pricing and Cost Pressure?

Acute care same-facility net revenues rose 4.7% to $2,610.7 million as adjusted admissions grew 4.9%, but revenue per adjusted admission fell 1.7% and the pre-tax margin narrowed to 11.6% from 12.8%.

Behavioral health same-facility net revenues rose 4.0% to $1,857.4 million, with adjusted admissions up 2.5% and revenue per adjusted admission up 1.7%. Its pre-tax margin eased to 18.1% from 19.2%.

One-Time Items: How Much Did Medicaid Payments and Litigation Reserves Move the Quarter?

The quarter included a $47 million favorable pre-tax effect from Medicaid supplemental payments, net of provider taxes, mainly in Washington, D.C. and California. It also carried a $26 million pre-tax increase in reserves for self-insured professional and general liability claims, tied mainly to a tentative settlement of the Cumberland Hospital litigation reached in August.

The net effect was about $21 million favorable before tax, versus about $55 million a year earlier, so the comparison base was higher. Settlement terms are confidential and court approvals are pending, with a substantial portion covered by excess insurance.
